What are Nuneaton and Bedworth PCN ARRS Roles?

Nuneaton & Bedworth PCN has a range of additional roles supporting all surgeries within the Primary Care Network allowing for specialised patient care.

 

 

Care Coordinators

Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

The care coordinator role ensures patient health and care planning is timely, efficient, and patient-centered; this include responsibilities for the coordination of the patient’s journey through primary care and secondary care.

This is achieved by bringing together all the information about a person’s identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person.

The care coordination role is embedded within the Primary Care Network Direct Enhanced Service, which aligns to the NHS Long Term Plan’s commitment to make personalised care business as usual a cross the health and care system.

Clinical Pharmacists

Clinical pharmacists are experts in medicines and how they are used. We are very fortunate to have clinical pharmacists in our team as they can help our doctors and nurses to spend more time with patients while they deal with the majority of medicine-related queries.

Our clinical pharmacists deal with repeat prescriptions, reviews, common minor ailments, urgent medication requests and deal with medication for patients recently discharged from hospital.

Our GPs may ask the clinical pharmacist to get in touch with you if you take lots of medications from time to time. This is to review your symptoms and check they’re still the best medications to be on. Where better, newer alternatives are available, they’ll discuss this with you and your GP.

Connect and Learn (Digital Inclusion)

Coventry & Warwickshire Cooperative Development Agency (CWCDA) and Nuneaton & Bedworth Primary Care Network have worked in partnership to provided dedicated face to face training and support, assisting the local community with access to NHS online services.

Would you like help in using the NHS App? Learn how to book an appointment online? Or even gain the confidence to use online banking, change device settings, or complete an online job application? The Connect & Learn Sessions are here to support you.

The sessions are FREE, available to all, and you do not need to book! The sessions are available for up to 10-weeks, so you can attend as many as you need, or just simply drop-in at your nearest venue!

Dietetics

The PCN currently employs a small team of dietitians.

What is a dietitian?

Dietitians are qualified healthcare professionals who use nutrition science to help people make informed decisions about the food they eat and their lifestyle, both for promoting good health and improving symptoms of disease.

We use the most up-to-date public health and scientific research on food, health and disease which we then translate into practical guidance to enable people to make appropriate lifestyle and food choices.

We provide individual advice to adults with the following conditions:

Malnutrition
Diabetes/high risk of developing Diabetes
Weight Management
Cardiovascular Disease
Functional bowel disorders

We offer a combination of face to face and telephone clinics. Currently face to face clinics are based at:

Enhanced Access

Enhanced Access providing patient access to appointments is being delivered directly by Nuneaton and Bedworth Primary Care Network on behalf of its 16 General Practices from April 2024; the service will offer improved access to appointments and other general practice services, virtually and face to face when your Practice is closed.

This ‘enhanced access’ means:

  • GP services will be providing appointments until 8pm on weekdays, and from 9am-5pm on Saturdays as a minimum.
  • Appointments will be both bookable in advance and available for same day (and able to be cancelled by the patient).
  • A mix of services will be available within these additional hours with access to the range of health professionals working in the primary care team.
  • There will be a choice of ways to access appointments depending on clinical need, such as telephone, online consultation, or face to face.

If a face-to-face appointment is needed, this will be at one of two GP practices in the local area.

Practitioners will include General Medical Practitioners (GP), Nurses, Advanced Nurse Practitioners (ANP), Advanced Clinical Practitioners, and Health Care Assistant’s (HCA). As the service develops it is hoped to also include First Contact Practitioners, Pharmacists and Pharmacy technicians too.

Appointments will be available to book 14 days in advance.

The Enhanced Access service is to compliment you general practice delivery and will be run ‘initially’ virtually weekdays (Monday – Friday) between 18.30 and 20.00 and face to face on Saturdays from 09.00 – 17.00.

First Contact Physiotherapists

Our First Contact Physiotherapists are here to assess and diagnose any new aches, pains and injuries to your bones, joints and muscles. 

They offer expert advice on how best to manage your condition, arrange follow-up appointments and can refer you to a specialist for further treatment if necessary. They can also speak to the GP straightaway if you need any medication during your appointment.

Physiotherapists train for longer than GPs on issues with bones, joints and muscles so are the perfect specialists to see for these matters. Having a physiotherapist directly at your GP practice means you get seen quicker and recover faster. 

Menopause Support Clinics

Between October 2024 – October 2025

We are running a monthly drop-in evening clinic for patients registered at any of the GP practices across Nuneaton & Bedworth. This is an opportunity for ladies going through the menopause to share experiences and link in with each other. The clinics will be held at The Grange Medical Centre, Nuneaton CV11 6LB from 7.00pm – 8.00pm and consist of a presentation from a GP for about an hour and then offer refreshments and an opportunity for attendees to share experiences and link in with each other.

 

 

Mental Health Link Workers

Living Well Mental Health Practitioners do mental health support differently, with a focus on your skills, aspirations and experiences.

We support people who experience mental health difficulties to work towards recovery, stay well, make their own choices & take part in everyday life.

Living Well mental health practitioners are available across all practices.

They work closely with our GPs and can talk to you about the wide range of treatments that can help you. This may include medication options, therapy or support with relationships, housing, debt or employment.

Whether you’re experiencing new or existing mental health problems, our Mental Health Practitioners are here to offer support that stops difficult times from becoming worse.

Pain Management Service – Rehab Direct

What is persistent pain?

Chronic or persistent pain refers to pain that lasts for more than 3 months, despite medication or treatment. Unlike acute pain, which serves as a warning sign of injury or illness, chronic pain persists after the initial cause has healed. Sometimes, pain can manifest without any history of an injury or operation.

Chronic/persistent pain can significantly impact your life, both physically and emotionally. It can:

  • Limit your ability to perform daily activities
  • Interfere with sleep
  • Lead to fatigue
  • Affect mood and mental health wellbeing
  • Strain relationships

What is the Pain Management Service?

The remote service provides easy, direct access to experienced professionals who are specifically trained in pain management.

This service is appropriate for patients over the age of 16 presenting with pain that has persisted for 6 months or more.

The programme aims to:

  • Support you with specialist Pain Practitioners and personal Health & Wellbeing Coaches
  • Help you understand how chronic pain develops and persists
  • Show you ways to manage your pain
  • Support your independence
  • Improve your quality of life
  • Help find the best medicines for your pain

Patients are offered a 16-week programme where they can explore a range of modules and tools to help manage their own pain. This service is not suitable for patients awaiting investigation or treatment for their pain, or patients experiencing complex mental health issues.

Safeguarding Information

Reporting an Adult Safeguarding Conce

I feel I am being abused or neglected, how can I report this?

  1. Tell someone you trust immediately; this might be:
    • a family member
    • a friend
    • a visiting professional such as a nurse or care worker
    • your family doctor (GP)
    • a hospital doctor or nurse
    • a Police officer
    • a housing officer

All of these people will be able to help you. They will pass on the information you give them to someone who can help or will help you to pass this information on yourself.

They will not do anything which will make things worse for you and your views, wishes and feelings will always be considered.

2. Where an adult is:

  • Unable to look after themselves, due to the level of their care and support needs, and
  • Is experiencing or is at risk of abuse or neglect

Making a Safeguarding Referral for Children:

Before making a referral – please take a look at the Spectrum of Support document to decide whether your concerns require a referral to Children’s Social Care.

Social Prescribing and Health Improvement

Our social prescribing link workers and health improvement team help with a whole range of non-clinical needs that can have a profound effect on your health and wellbeing. They have time to listen and work with patients over a number of weeks both face to face and by phone/video.

They can put you in touch with community support and groups, as well as come along with you to get you started. They can help you plan what you want to do and get there.

Although they’re unable to do it for you, they’ll be there for you the entire way, listening to your needs and providing all of the information that organisations and groups can do to help you.
